Pernicious anemia or other anemias due to vitamin B12 deficiency (due to potential masking of deficiency with leucovorin)
Patients with known glucose-6-phosphate dehydrogenase deficiency
Patients with known glucose-6-phosphate deficiency, psoriasis, porphyria and psychosis
Known glucose-6-phosphate dehydrogenase (G-6PD) deficiency

Known or documented bleeding disorders not limited to: anti-phospholipid syndrome, homozygotes for Factor V Leiden deficiency, antithrombin III deficiency, protein C deficiency, protein S deficiency, hyperhomocysteinemia, systemic lupus erythematous, or prothrombin G2020 gene mutation
History of B12 deficiency
