Evidence of serious suicide risk including any history of suicidal behavior in the last months and/or any suicidal ideation in the last months and/or poses a significant suicide risk in the judgment of the investigator
Recent (within the past year) or active suicidal ideation or behavior
Suicidal or homicidal ideation.
Suicidal ideation or history of suicide attempt
Patient has a medically documented history of or active major depressive episode, bipolar disorder (I or II), obsessive-compulsive disorder, schizophrenia, a history of suicidal attempt or ideation, or homicidal ideation (e.g. risk of doing harm to self or others); ), or patients with active severe personality disorders (defined according to Diagnostic and Statistical Manual of Mental Disorders [DSM]- IV) are not eligible; patients who have a history of major depression that is controlled with chronic oral therapy are eligible provided they have no history of inpatient hospitalization, history of documented suicide attempt or ideations, and are managed with one anti-depressant which has not required dose adjustment in weeks; patients with a history of depression who would otherwise be eligible should be evaluated by a mental health professional prior to enrollment if there is any uncertainty regarding the status of their mental health; Note: for patients with psychotropic treatments ongoing at baseline, the dose and the schedule should not be modified within the previous weeks prior to start of study drug
History of psychiatric disorder (e.g. depression; suicidal ideation; psychosis)

Any other condition which, in the opinion of the Investigator, might impair the subject's tolerance of trial treatment, the safety of the individual subject or the outcome of the trial. (including but not limited to: history of major depressive episode, bipolar disorder, obsessive-compulsive disorder, schizophrenia, suicidal attempt or ideation, homicidal ideation, or >= Common Terminology Criteria for Adverse Events [CTCAE] Grade anxiety)
Any patient with a history of major depressive episode, bipolar disorder, obsessive/compulsive disorder, schizophrenia, a history of suicide attempt or ideation, or homicide/homicidal ideation as judged by the investigator and/or based on recent psychiatric assessment may not participate in this study without discussion with and agreement of the study principal investigator (PI)

Active suicidal ideation with plan and intent
Psychiatric condition that would preclude or take precedence over study participation (e.g., active psychosis, suicidal ideation)
Subjects who are actively suicidal or homicidal
Have a psychiatric diagnosis that would require significant study modification to meet their needs such as uncontrolled severe mental illness, substance abuse, or active suicidal ideation
PATIENT: No evidence of thought disorder, delusions, or active suicidal ideation is observed or reported
PATIENT: Evidence of thought disorder, delusions, hallucinations, or suicidal ideation
CAREGIVER: Evidence of thought disorder, delusions, hallucinations, or suicidal ideation
Patient has serious psychiatric condition (e.g., bipolar disorder, schizophrenia or other psychosis, bulimia or anorexia nervosa, suicide attempt within months or current active suicidal ideation) that would compromise the patients ability to complete the study, at the discretion of the investigator
Previous history of suicidal ideation, homicidal ideation, depression leading to hospitalization or mood disturbance leading to hospitalization
Medical record documentation of severe mental illness (i.e., schizophrenia or bipolar disorder), active suicidal ideation, or active substance use disorder
Has unstable suicidal ideation as determined by the patient's treating psychiatrist
Acute suicidal behavior.
Current suicidal ideation or suicide attempt within past months
Overt cognitive dysfunction or psychiatric disturbance such as suicidal ideation or severe mental illness, as observed or judged by the researcher, referring source, or other qualified observer
Participants with major depressive disorder and/or suicidal ideation as determined by PHQ-
Subjects who are actively suicidal or homicidal
Endorsing current suicidal ideation or intent
Unstable psychiatric/medical conditions such as suicidal ideation, acute psychosis, severe alcohol dependence, or dementia
Has Suicidal Risk Assessment (SRA) scores >=
Severe psychiatric illnesses (current schizophrenia, major depression with suicidal ideation, or substance abuse within two years)
