EXCLUSION CRITERIA FOR SECOND-LINE THERAPY: Cardiovascular disease problems including unstable angina, therapy for life-threatening ventricular arrhythmia, or myocardial infarction, stroke within the last  months, or a diagnosis of congestive heart failure
