Has been administered a live vaccine within  weeks ( days) of initiation of study therapy. Patients are eligible if  days have elapsed since receipt of vaccine and initiation of study treatment. (NOTE: seasonal vaccines for influenza are generally inactivated vaccines and are allowed. Intranasal vaccines are live vaccines; and are not allowed).
Has received a live vaccine within  days of planned start of study therapy; (Note: seasonal influenza vaccines for injection are allowed as they are inactivated; however, intranasal influenza vaccines are live attenuated vaccines and are NOT allowed)
Has received a live vaccine within  days of planned start of study therapy\r\n* Seasonal influenza vaccines for injection are generally inactivated flu vaccines and are allowed? however intranasal influenza vaccines (e.g., FluMist) are live attenuated vaccines, and are not allowed
Clinically significant cardiac disease, including unstable angina, acute myocardial infarction within  months from day  of study drug administration, New York Heart Association class III or IV congestive heart failure, and arrhythmia requiring therapy\r\n* Note: Seasonal influenza vaccines for injection are generally inactivated flu vaccines and are allowed; however intranasal influenza vaccines (e.g., Flu-Mist) are live attenuated vaccines, and are not allowed within  days prior to initiation of treatment
Has received a live vaccine within  days of planned start of study therapy (cycle , day )\r\n* Note: The killed virus vaccines used for seasonal influenza vaccines for injection are allowed; however intranasal influenza vaccines (e.g., FluMist) are live attenuated vaccines and are not allowed
Has received a live vaccine within  days prior to first dose of therapy; Note: seasonal influenza vaccines for injection are generally inactivated flu vaccines and are allowed; however intranasal influenza vaccines are live attenuated vaccines, and are not allowed
Patients who have had influenza, hepatitis, or other vaccines within a month prior to initiation of study drugs
