Absence of history of congenital long QT syndrome
Long QT syndrome or family history of idiopathic sudden death or congenital long QT syndrome.

Resting heart rate < bpm or > bpm Long QT syndrome or family history of idiopathic sudden death or congenital long QT syndrome, or any of the following:
